This happens in the 2nd room, when you enter the down stairwell on the same frame as when the door to the right opens (when you kill all the enemies in the room). The best way to prevent this is to not kill all the enemies in the room before entering the stairwell.
The fastest solution is to mirror immediately once you notice this happen.
